Our chronic disease nurse is specially trained to help optimize the quality of life for patients and their caregivers, young patients or parents with a child living with a chronic respiratory disease like emphysema and asthma, commonly called COPD (Chronic Obstructive Pulmonary Disease).
Our program aims to:
- Reduce your symptoms
- Increase your participation in self-management
- Delay or prevent hospitalization or emergency room visits
- Improve fitness & physical condition
- Promote lifestyle changes
A variety of services are offered to you:
- Educational sessions (i.e. disease process, technique for inhaled medications, action plan, and stress management)
- Smoking cessation counseling
- Spirometry test (lung function test) to screen or to monitor the disease
- Supervised individualized exercise program (1-2 times/week)
- Nutritional assessment, if needed
The following multidisciplinary personnel will work with you:
- Chronic disease nurse
- Health promoter
- Mental health counselor
- Pharmacist
- Nurse practitioner and/or Dietitian.
To receive these services, you must be a resident of the Province of Ontario and be referred by a family physician who is a member of our Family Health Team.
